Contracted Plans: Our office will file your claim with your insurance company if we are contracted with your plan. We require deductibles, co-pays, and any percentages to be paid at the time of your scheduled appointment.
Non-Contracted Plans: Payment in full is required at the time of your scheduled
appointment. If we are not contracted with the insurance plan, we will file a completed claim to the insurance company as a courtesy, and the insurance company will forward the reimbursement directly to you, the patient.
Bring your insurance card and any policy information to your visit. Each insurance policy is different and reflects a contract between you and your insurance company. You must also be aware of the restrictions of your managed care health plan. This may affect referrals and any ancillary services (labs/x-rays). In order for your service to be covered, only certain laboratories or x-ray facilities may be used.
While we will be able to assist you with many of these questions, the final responsibility for payment lies with you, the insured.
